| Association Membership | The Association, Peoples' Benefit Alliance, is a membership organization that provides educational information and discounts on goods and services to its members. Membership in the Peoples' Benefit Alliance is required, at an additional cost, in order to be eligible to apply for a Dental Preventive Plus plan. The Association is only required in Arizona, Michigan and Wisconsin. This fee will be waived if the Dental Preventive Plus plan is purchased in conjunction with an association based medical plan. |

The Dental Preventive Plus effective date will be the first of the month following the issuance of your medical policy and may differ from your medical effective date. All billing and payments will be separate from your medical plan and will occur through HumanaOne Dental, a Humana company.
