HumanaOne's Dental Preventive Plus option encourages preventive treatment to help you and your family achieve good dental and overall health. This optional dental benefit is ideal for those looking for preventive dental coverage along with their health insurance coverage from Humana. Enrollment in this plan requires a minimum one-year contract.

Preventive Services |
Covered at 100% when you visit an in-network dentist with no deductible or waiting period. Coverage includes:
- Oral exams
- Cleanings
- X-rays
- Sealants(through age 14)
- Topical flouride treatment (through age 14, one per calendar year)

Basic Services |
Covered at 50% after meeting deductible when you visit an in-network dentist. Waiting periods apply. Includes:
- Emergency care for pain relief
- Extractions and root removal
- Fillings (amalgam, composite anterior teeth)
- Oral surgery
- Space maintainers
- Prefabricated stainless steel crowns

Discount Services |
Receive discounts on services not covered under this plan when you see a participating dentist. Out-of-pocket expenses do not apply to deductible or annual maximum.

- Appliances for children
- Endodontics (root canals)
- Periodontics

- Dentures
- Denture repair and adjustments

Association Membership |
The Association, Peoples' Benefit Alliance, is a membership organization that provides educational information and discounts on goods and services to its members. Membership in the Peoples' Benefit Alliance is required, at an additional cost, in order to be eligible to apply for a Dental Preventive Plus plan. The Association is required in all states except Colorado, Georgia, Kentucky and Utah. This fee will be waived if the Dental Preventive Plus plan is purchased in conjunction with an association based medical plan.

The Dental Preventive Plus effective date will be the first of the month following the issuance of your medical policy and may differ from your medical effective date. All billing and payments will be separate from your medical plan and will occur through HumanaOne Dental, a Humana company.
