Specialized Care:If you are enrolled in a plan that requires referrals for specialized care, you’ll need a referral to use your benefit most effectively. When you need specialist care, your primary care physician will refer you to a specialist or hospital he or she routinely uses. Many primary care physicians limit their referrals to selected specialists and hospitals, so be sure to check with your doctor whenever you need a referral. You should not assume that the services of a particular specialist or hospital listed here will be covered according to your benefits, unless you have the appropriate referral. Sometimes state or federal regulations may give you an even broader range of doctors to choose from – for example, if you live in Texas , you can designate an obstetrician or gynecologist without a referral. Consult your plan documents for more information.
Copayments:Copayment amounts for in-network doctors and other providers may vary depending upon the benefit plan you have chosen. Please check your Certificate of Coverage or Plan Benefit Document to see if your plan has different provider copayment levels. You may also check the copayment schedule by calling the number on your ID card.
Transplants:The providers listed in this directory are participating for general medical services. However, these providers may not be participating for transplant services. Use of a general medical service hospital for transplant services may result in substantial additional expenses to you. Physician Billing Practices:We suggest you contact your provider's office to inquire on their particular billing practices, as some providers may bill their services as outpatient hospital versus clinic. Please refer to your Benefit Plan Document to verify coverage, as Humana has no control how a provider bills.
