Dental HI215 DHMO

With the Dental Prepaid HI215 plan, you choose a primary care dentist from our network to provide your dental care. For every office visit, you pay a low $15 copayment and you don't need to submit claims. There are no yearly maximums, deductibles, or waiting periods. This plan carries a one year contract. Price quoted includes all recurring fees. Please note that your initial payment will also include a one-time, non-refundable $35 enrollment fee. Benefits include:

  1. Preventive services such as cleanings, X-rays and oral exams, are covered at no additional cost after the $15 copayment
  2. Basic and major services including fillings, crowns, root canals, dentures, and many other common procedures, are covered with a copayment that varies by procedure
  3. Discount – You may receive a discount up to 25 percent when you visit any specialist in the plan’s network, including Orthodontists
