HumanaOne's vision option can help reduce the cost of visiting a vision care doctor.
| Annual Examination | $10 copayment for your annual, comprehensive vision examination This thorough examination is designed to evaluate not only your vision, but overall eye health as well. |
| Frames | $15 copayment for new frames once every 12 months |
| Network | Our large network of private practice vision care doctors lets you enjoy the comfort and privacy of visiting a private doctor's office for your vision care. Use our Provider Finder to see if your vision care doctor is in-network. |
| Lenses | You will receive lenses at a 20% discount from the normal charge |
| LASIK | LASIK is offered for plan members who are nearsighted or have astigmatism and wear glasses or contacts. We are contracted with approximately 150 premier LASIK facilities, including The Laser Center (TLC) doctor network, to offer this procedure at substantially reduced rates. Members receive benefits when services are received from preferred TLC Vision LASIK Advantage Centers at the following fixed prices: • Conventional LASIK $895 per eye • Custom LASIK $1,295 per eye • Custom LASIK with IntraLase $1,895 per eye If you choose another participating location, you will receive a 10% discount from the provider and pay no more than $1,800 per eye for Conventional LASIK procedures and $2,300 per eye for Custom LASIK. |
| Discounts | You will be given a 20% discount on a second pair of eyeglasses and/or a 15% discount on professional service fees for elective contact lenses such as fitting and follow-up exams. These discounts are available for 12 months after you receive a covered eye exam from a network doctor. |
| Additional Fees | A one-time, non-refundable, $35 enrollment fee applies to your initial payment. A $1 administration fee also applies per bill, but it is waived if you pay yearly. |
