If you want coverage for many of the same services as a 100% After Deductible plan – plus the ability to have a healthcare spending account – consider our HSA-Qualified High Deductible Health Plan.

This plan may be a good fit for you if:

You already have a Health Savings Account (HSA) or you want to open one: The HSA is a tax-free way to budget and pay for your deductible and other qualified medical expenses (varies by state; please contact your local tax advisor for tax deductibility).
You want a simple plan design: Once you meet your deductible, the plan pays most covered in-network medical expenses at 100%.

Individual coverage annual deductible options (combined medical/prescription): $1,500 | $2,500 | $3,500 | $5,000 | $5,950
Family coverage annual deductible options (combined medical/prescription): $3,000 | $5,000 | $7,000 | $10,000 | $11,900
For family coverage, all covered members' covered expenses apply to the family deductible; for example, if the expenses for one member are enough to meet the family deductible, the rest of the family doesn't need to meet an individual deductible
In-network illness/injury doctor's office visits, inpatient hospital, and outpatient hospital services are paid at 100% after deductible for most covered medical expenses
Includes prescription coverage
