  • Preventive care services covered at 100%, when you use an in-network provider
  • Inpatient and outpatient hospital services paid at 80% after deductible for most covered medical expenses

  • Up to six illness/injury in-network doctor's office visits, and urgent care for a copay per visit
  • Preventive care services covered at 100%, when you use an in-network provider
  • Inpatient and outpatient hospital services are paid at 80% after deductible for most covered medical expenses
  • Includes prescription coverage
