With HumanaOne, you can “build your own” plan by choosing the benefit options* that are right for you and your family.

Dental Benefit
This benefit includes coverage for preventive care services, basic and major services, teeth whitening services, and discounts on orthodontia services. You can also keep your dental coverage if you cancel your medical policy or move within the United States. Learn more.

Lifetime Maximum Benefit
Many HumanaOne plans pay a maximum of $2 million in total benefits over the lifetime of the policy. But if you want even greater protection, you can opt to increase to a larger lifetime maximum amount such as $5 million or $8 million depending on the plan you choose.

Office Visit Copayment Benefit
Adding this coverage option means that your plan pays 100 percent of in-network office visits and 60 percent of out-of-network office visits after a copayment. This includes diagnostic tests, labs, and x-rays paid 100 percent up to $100 per calendar year. Four visit per year per member. For Arizona and Utah residents this benefit is included in the HumanaOne plan and visits are not limited.

Primary/Spouse Life Insurance Rider
Provides $20,000 of life insurance coverage for the primary applicant and/or the applicant's spouse. This coverage terminates when the health plan terminates.

Supplemental Accident Benefit
Adding this benefit option means you're eligible for coverage before meeting your deductible -- up to $1000 per incident. This inexpensive additional coverage provides extra protection in case of minor accidental injury.

HumanaOne Term Life Insurance
Affordable financial protection with coverage amounts from $25,000 to beyond $1 million and premiums are level for 10, 15, or 20 years guaranteed to age 95.

Zero Deductible Prescription Benefit
With this coverage option, you don’t have to meet a separate $500 prescription deductible before your pharmacy benefits begin.
